Incoming freshman

 

All incoming 9th graders are required to have a physical examination completed on the mandated Illinois form by a licensed physician and proof of adequate immunizations (The parent portion must be completed, signed and dated).

Students New to the District

Students entering from out of state are required to have:

1. A physical examination completed on the state mandated Illinois form by a physician (the parent portion must be completed, signed and dated) Click for Form

Students entering the district from another Illinois school are required to show proof of:

1. Having had a completed 9th grade physical or more recent on the state mandated Illinois form by a physician.

Medication Administration Requirements at School 


ALL medication is to be kept in the clinic and must be brought/picked to/from the clinic up by the parent/adult. 

 A med form is required for the administration of medication without parent/guardian present.  Over the counter medication should be brought in an unopened package and only requires the signature of a parent.  Prescription medication must have a med sheet completed and signed by the student’s physician and signed by parent/guardian. Medication Administration Form

Student using inhalers for asthma control, Epi-pens for allergic reactions, and diabetic glucose monitoring must have the physician’s written authorization to “self-administer” their medication or carry it on their person at school. Self Administration Medication Form
