Payroll and Leave Requests

All vacation time must be pre-approved by the building principal and the human resources department.

When taking sick or personal day:

Call the absentee line (309) 743-8836.

Leave a message stating your name, the building where you work, your shift, and the reason for your absence.

All vacation time must be pre-approved by the building Direct of Facilities and the human resources department.

**MHS custodians are to turn their paperwork into the Administrative Assistant for Moline High School.

All other custodians turn their paperwork into their building principal for approval and then to the building administrative assistant for payroll purposes.

Please make sure the Facilities Administrative Assistant to the Director of Facilities, receives a copy of all paperwork.

Custodial/Maintenance (AFSCME) Request To Be Absent (Electronic Signature)

Custodians & Maintenance use this form to request regular sick, vacation, personal, FMLA, and bereavement leave. This form is routed to the appropriate department supervisors and payroll processors.

Association Day Request Form for MESPA, AFSCME, and MEA (Electronic Signature)

All union groups use this form to request an Association Day. This form is routed to the union leader, then to the building administrator.

Monthly Inspection Checklist - Please submit the request to have the form emailed to you to complete. Once it is submitted administration will receive it.

Water Meter Reading - Email to Facilities Administrative Assistant on the 15th of each month. Keep a copy on file at your location for examination by the state fire marshal.
