We accept private insurance, Illinois public aid, and we also offer a sliding fee based on your family size and income.

Moline School District Physical and Immunization Clinics
Moline High School: October 6th
John Deere Middle School and Wilson Middle School: October 13th
School Health Link will be providing physicals and giving immunizations to Moline students, on campus during school hours, October 6th and 13th. If your child attends Moline High School, John Deere Middle School, or Wilson Middle School, and you interested in participating, please print and complete these required documents. Documents should be returned to your child's school nurse no later than 1 week prior to the scheduled clinic.
